Section Two - Participants Pre-requisites:  Current Military ID , (must be a spouse or dependent of active duty service member)  Must be 18 years old before the first day of class  Immunizations must be current Section Three - What is expected of the Volunteers To ensure a productive and quality instructor-to-student ratio, class size will be limited to a minimum of three with a maximum of six students. Therefore, it is imperative that the participants understand the scope of the training and commitment that is required: A. CPR/First Aid certificate must be presented on the first day of class. (Red Cross will provide CPR training to all participants.) B. Classroom: 80 hours of mandatory classroom instruction and a score of 80% or higher on test. To successfully pass the classroom portion and move on. Failure to attend all classroom sessions will result in dismissal from the program regardless of test scores. C. Chair-side: 600 hundred hours of chair-side practical experience to be completed within six to nine (6-9) months. The expectation for all students is at least 25 hours per week. This is necessary because students are required to train in a variety of areas including clerical, medical records, x-rays, sterilization, and oral surgery. Additional information: 1. This course, both classroom and chair-side will be conducted in English. 2. During chair-side portion, any DAP participant who does not volunteer for three consecutive weeks will be automatically disqualified from the program unless a verifiable emergency exists. 3.
